Persons with diabetes mellitus type 2 are exposed to a greater medical risk of suffering from cardiovascular disorders. To counteract this risk, we advise you and provide therapy that follows an evidence-based care concept.
The concept focuses not only on medical factors but also treatments such as physical activity and nutrition. Cooperation between the professional groups involved is needed for the care concept to be implemented successfully: Medical professionals, physiotherapists, specially trained medical assistants and nutrition and diabetes experts. The interdisciplinary organisation of our Medical Center ensures this.
Process
The patient formulates their goals together with their general practitioner and, if required, other experts. This process is repeated together with the patient in an annual GP follow-up. The goal setting is followed by an action catalogue, which is also developed together with the patient.
